 DOH / HAAD exam registration prosses :

1. Submit the request through the available channels .
2. Create unique identification number through DoH user management system .
3. Visit data flow website and submit all the requirements and documents for primary source verification .
4. Receive notification of the completion of the verification process .
5. Review your application prior to submitting it to the credentialing process through User Management System (UMS) .
6. Pay the application fees .
7. Credentialing and review process for license eligibility determination .
8. Pay the issuance fees and print the license electronically upon the completion of payment .

1. Attested Certificate / Academic Qualification .
2. Mark Sheet .
3. Emirates ID If available .                                                                          
4. Experience certificates .
5. Good Standing Certificate .
6. Form No. 1 Staff data form .
7. Form No. 2 E-tarasol barcode .
8. Copy from your passport .
9. Passport size photo recent two years.
